1555 Soquel Drive, Santa Cruz, 95065 The Diabetes Management Program provides information and support to help create a personal, successful path to diabetes control.The program includes education classes and individual appointments provided by or team of health care professionals including certified diabetes educators, registered nurses, and registered dietitians. ELIGIBILITY: Diagnosis of diabetes or pre diabetes. Significant others and family are welcome to attend appointments along with the person receiving the service. FEES: Fixed fees. Insurances may reimburse for this service. | ||
| Programs: | CLASSES: Overview of Diabetes: An introduction to diabetes as a chronic disease, which can be managed with proper treatment. Review of signal symptoms, types of diabetes and risk reduction. Nutrition and Diabetes: A review of the effect of food on blood sugar. Emphasizes strategies on controlling blood sugars by making diet lifestyle changes. Regulating Blood Glucose & Exercise Guidelines: Learn normal and abnormal blood glucose ranges and intervention strategies for maintaining tight glucose control. Review how to maintain the proper balance of blood glucose with diet, exercise and medication.
INDIVIDUAL CONSULTATION: Glucose Monitor Training: Get the latest information about glucose monitors and which monitor is best for your needs. Learn the proper technique for testing your blood sugar and using your glucose monitor. Insulin Injection Training: Introduction to correct injection sites, injection tips, and how to store and handle your insulin. Meal Planning: Receive individualized meal plans and develop nutrition goals specifically designed for you. Carbohydrate Counting: Help in developing an individualized carbohydrate and insulin ratio for those who are insulin dependent. Consultations are 30-90 minutes. | ||
