| Description: | The Child Health and Disability Prevention (CHDP) is a preventive program that delivers periodic health assessments and services to low income children and youth in California. CHDP provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services. Health assessments are provided by enrolled private physicians, local health departments, community clinics, managed care plans, and some local school districts.

| Mission: | The Santa Cruz County Child Health and Disability Prevention (CHDP) program is dedicated to the health and well being of children. As part of the State Children's Medical Services, CHDP provides for the early detection and prevention of health problems among children from low-to-moderate income families. Program staff works collaboratively with health care providers and child advocate agencies to ensure that eligible children receive quality health care and to empower families to be knowledgeable, responsible health care consumers. | ||
| Programs: | Through the CHDP Program, children and youth can obtain regular preventive health assessments to identify any health problems. An assessment includes a full review of the child's medical and developmental history with screening of certain medical items, including all childhood immunizations. Those with suspected problems are then referred for necessary diagnosis and treatment. Many problems can be prevented, corrected, or the severity reduced by prompt diagnosis and treatment. The earlier they are found, the more easily they can be treated or corrected.
The program also refers eligible children on to other health insurance plans such as Healthy Families, Medi-Cal and Healthy Kids of Santa Cruz County. | ||
