| Description: | A community service program which provides support for individuals with a life limiting illness and their loved ones. Transitions helps to ease concerns and identify community resources to assist with health care issues. Medical Social Workers maintain a connection throughout the program and guide individuals and their loved ones as they adjust to living with a life limiting illness. Health care options, advance planning to clearly state end-of-life wishes, and other practical needs can be explored and discussed together.
ELIGIBILITY: Available to anyone with a serious illness or those wishing to explore future healthcare decisions. FEES: Sliding scale based on adjusted monthly income. | ||
