Community Connection (CC)is a nonprofit mental health program of the Volunteer Center of Santa Cruz County that serves adults and families referred through the Santa Cruz County Mental Health System of Care. Our mission is to assist and support adults with psychiatric disabilities and their families in achieving greater independence and improving the quality of their lives. Participants have access to a variety of person-centered, strength based programs that provide pre-employment, employment, education, community integration and peer support services for individuals and families. ELIGIBILITY: serves residents of Santa Cruz County who are at least eighteen years of age, have a severe disability in at least one functional area and have been diagnosed as schizoaffective, schizophrenic, bipolar or major depressive by County Mental Health. Services will be provided to all individuals meeting these eligibility guidelines without regard to ethnicity, religion, age, disability, national origin, gender, or sexual preference. FEES: Medi-Cal, MediCare, sliding scale according to fee evaluation by County Mental Health. No one will be denied services because of inability to pay FUNDING SOURCE: Community Connection is funded in part by Santa Cruz County Mental Health Services, State of California Department of Rehabilitation and United Way of Santa Cruz County. | ||
| Programs: | Community Connection offers a variety of free, outpatient mental health programs and employment related services to adults with psychiatric disabilities and families residing in Santa Cruz County.
Programs: Career Services provides support and assistance to individuals referred through County Mental Health who are interested in gainful employment in the community.
