Description:Hospice of Santa Cruz County offers compassionate, expert medical care and grief support to patients and their loved ones facing the end-of-life. Caring for the body, mind and spirit, we support the unique needs of each patient in the comfort of their own home, in a nursing home or in a residential care facility. We are experts at symptom management so that individuals can live in comfort and dignity. We offer support to patients and their families 24 hours a day, seven days a week. Our Transitions program supports those with a life-limiting illness by easing certain concerns, creating care giving plans, and identifying community resources to assist with healthcare issues.

ELIGIBILITY: Determined case by case.

FEES: Hospice care is covered by Medicare, most insurances or a sliding scale.

Programs:Grief support is available for hospice families and grieving community members in group and individual settings. HUG (Healthy Understanding of Grief) Program helps children process their grief in a healthy, supportive and age-appropriate way. Please call 430-3000 for our current schedule.

A Drop In Grief Support Group meets every Tuesday at 6:00pm in Aptos, please call (831) 430-3000 for directions. Join with other adults grieving the death of a family member or friend. Share stories, learn tools for coping, and receive support from people who care. No registration required.

An additional Drop In Grief Support Group facilitated in Spanish is held each Monday, 4pm at Catholic Charities, 217 E. Lake Ave., Watsonville. Offered in conjunction with Catholic Charities of Monterey, the group provides a safe place to share thoughts, feelings and memories in a supportive environment. HUG - Our Healthy Understanding of Grief program for children and adolescents offers individual, group and in-school support. Kids receive age-appropriate support and tools to process their grief in a healthy manner.
Support in the workplace for coping with a death or anticipated death.
A Lending Library of books and tapes on grief and loss.
Referrals to other community resources.
